Pediatric Reminder Pilot collects business contact details from people who ask to hear about the pilot. This notice covers the interest form only. Do not submit patient, child, health, or other sensitive information.

What we collect

We may collect your email address, name, organisation, role, appointment-volume range, current system, administrative challenges, preferred language, submission time, source, consent, and IP address.

Why we use it

We use this information to respond to your request, assess interest in the pilot, improve the proposed service, prevent abuse, and keep a record of consent. Our basis is your consent and our legitimate interest in operating and securing the pilot.

Where it goes

The information is stored with our hosting provider, Railway, and may be sent to Resend for signup notifications and contact-list management. We do not sell it.

Retention and your choices

We keep pilot-interest records while evaluating or running the pilot, then review and delete records that are no longer needed. You may ask to access, correct, delete, or stop marketing use of your information by replying to any pilot email you receive. You may withdraw consent at any time; this does not affect earlier lawful use.

Important limitation

This form is not a patient portal and must not be used for clinical or emergency information. This notice may be updated as the pilot develops.
